Provide the individual’s requirements and relevant information.

Initial Review
We consider whether Dayspring Centre can safely and appropriately meet the person’s needs.

Assessment
Where appropriate, an assessment is arranged with the individual and relevant representatives.

Support Planning
We agree desired outcomes, identified risks, preferences and support arrangements.

Service Commencement
Appropriate support arrangements are confirmed.

Review
Support is reviewed to ensure it remains safe, effective, responsive and outcome-focused.
